What is testicular hypofunction?

Testicular hypofunction, also called male hypogonadism, is the recognized medical condition in which the body does not produce enough testosterone. It is a real diagnosis with its own code (ICD-10 E29.1), not a lifestyle label, and it is diagnosed with blood work, not a questionnaire.

It comes in two forms

Primary hypogonadism starts in the testes themselves: they receive the signal to produce testosterone but cannot keep up. Secondary hypogonadism starts upstream, in the pituitary gland or hypothalamus, when the brain's signal to produce testosterone weakens. The distinction matters because it changes how your provider designs treatment, which is one reason a proper workup beats a quick prescription.

Why it becomes more common with age

Testosterone production typically declines about 1% per year after age 30. That slow slide is why symptoms often creep in gradually through your 30s, 40s, and 50s rather than arriving overnight. Standard reference ranges are built from broad population averages, typically spanning roughly 300 to 1,000 ng/dL. A 40-year-old whose level has drifted from 800 down to 320 is still technically "in range," yet his body is operating on far less testosterone than it was built for, and he often feels it: in energy, drive, mood, and body composition.

This is what low-normal testosterone means: levels near the bottom of the reference range, paired with real symptoms. What is testicular hypofunction?

Testicular hypofunction, also called male hypogonadism, is the medical condition in which the testes do not produce enough testosterone. It can originate in the testes themselves (primary) or in the brain's hormonal signaling (secondary). It is diagnosed with repeated morning blood tests alongside a symptom evaluation, and it is the formal diagnosis behind most medically supervised TRT.

My testosterone came back "normal." Could I still benefit from treatment?

Is TRT safe? What do you monitor?

Do I have to stay on TRT forever?

Testosterone deficiency is a recognized medical condition. It is important to have a thorough diagnostic blood work up to not only identify a deficiency, but to also look for potentially reversible causes and exclude possible contraindications.

The aim of Testosterone Replacement Therapy (TRT) is to correct the deficiency to levels where nature intended for men or women. This has traditionally meant administering testosterone monotherapy to help restore testosterone levels to within normal physiological parameters for long term physical and psychological health.

From a qualitative perspective, the aim of TRT is to reverse the negative symptoms associated with having a deficiency. Replacement can improve sexual interest and function, increase bone density and muscle mass, enhance cognition, maintain overall quality of life, strengthen cardiovascular health, and balance overall mood.

With any medical regimen, it’s important to be aware of side effects and risks.
